If my pet(s) should require veterinary care during his/her stay at Laurel Hill Kennels, I would like treatment to be administered by the staff of ________________________________________________. In the event of an emergency after my veterinarian’s regular business hours, I will permit treatment to be administered by the staff of the York of Westminster Emergency Clinic. I understand that these services will be at my expense.
 
I am aware that Laurel Hill Kennels makes every possible effort to maintain a healthy and safe environment for my pet(s) during his/her stay. Proof of current vaccinations is required of all boarders, and the pet’s area is kept free of any material that could be hazardous to my pet(s). I understand that allowing my pet(s) to be in close quarters with other pets and /or going out into an outdoor play yard comes with some risk. I accept this risk and will not hold Laurel Hill kennels responsible for the cost of veterinary services related to my pet’s stay at Laurel Hill Kennels. 

I understand that Laurel Hill Kennels makes every effort to follow my pet’s veterinarian’s directives in regard to my pet’s current medical condition. Many pets behave differently in a kennel environment, and this can impact the effectiveness and proper implementation of those directives. Therefore, Laurel Hill Kennels cannot be held responsible for any regression in my pet’s medical condition during his/her stay. 

I understand that Laurel Hill Kennels will not assume responsibility for my pet(s) beyond the agreed upon length to stay. If I do not contact Laurel Hill Kennels or cannot be contacted by Laurel Hill Kennels concerning an extended stay for my pet(s), Laurel Hill Kennels will maintain responsibility of said pet(s) for five consecutive days before relinquishing said pets to the York County SPCA.
